Miles of Smiles at Belle Valley South!!
Dear Parent/Guardian:
Miles of Smiles, Ltd. will be at Belle Valley South on Monday, March 15th. This is a mobile dentist unit that travels to schools through out Illinois.
Please look over the permission form, sign, and return it with your child as soon as possible. The dentist will see ANY student who has a signed waiver, including students with private dental insurance.
A dental exam is required for ALL students in the 6th grade. The exam that the dentist completes is sufficient for the state requirements.
